A case of primary percutaneous coronary intervention (PCI) is shown in the elderly patients with an ST-segment myocardial infarction. Stent deployment following the extensive thrombus aspiration resulted in a slow-flow phenomenon. Optical coherence tomography imaging showed a huge thin-capped fibro-atheroma encroaching the landing zone for stent placement. The author discusses possible factors involved in the development of the slowflow.
